For pre-approval for a Rehab Loan please fill out the application below:

Complete Application Packet.  

The Application can be faxed to 614-433-9190, emailed to info@ohiorehabloans.com, or mailed to 634 Bear Run Lane, Lewis Center OH 43035.  You can process your $50 application fee over the phone or include it with your mailed application.

Upon pre-approval the following additional documentation may be requested:

1. Last 6 months bank statements for both personal and business accounts - checking, savings, money market, cds, investment accounts, etc.

2. Tax Returns for last 2 years.

3. Income Verification (W-2s, 1099s, etc.)

4. Company documentation for any companies in which applicant holds an ownership interest. (TIN, letter of good standing from the Secretary of State, etc.)
